Option 1: Insurance

I am in-network with the following insurance plans: Aetna (PPO, POS, OAP, Select), Blue Cross Blue Shield/Anthem (PPO, EPO), Cigna (PPO, POS, OAP, EPO), Health Net of CA (PPO, Ambetter PPO, Ambetter HMO), Kaiser Permanente (PPO, HMO, Medi-Cal), Prime Healthcare EHP, and United Healthcare (PPO, EPO, HMO). If you have any of these insurances, please text or email me to request an appointment.

Option 2: Self Pay

If you don't have any of the insurances listed above, your appointment will be out-of-pocket. I will provide a Superbill for you at the end of our appointment to submit to your insurance for possible reimbursement. Please text or email me to request an appointment.

Home Visit - Initial: $200

Home Visit - Follow Up: $175

Telehealth - $175

*Payment is required at the time of booking the appointment.
