Option 1: Insurance

I am in-network with the following insurance plans: Aetna (PPO, POS, OAP, Select), Cigna (PPO, POS, OAP, EPO), Health Net of CA (PPO, Ambetter PPO, Ambetter HMO), Oscar, Prime Healthcare EHP, Tricare West, and United Healthcare. If you have any of these insurances, please request an appointment below.

Option 2: The Lactation Network (TLN)

I am partnered with TLN and they are in-network with most Blue Cross PPO, Blue Shield PPO, Anthem PPO, VA CCN, as well as most plans that utilize Multiplan and PNOA. Please contact me first so we can make sure you are covered first before scheduling your appointment.

Option 3: Self Pay

If you don't have any of the insurances listed above, your appointment will be out-of-pocket. I will provide a Superbill for you at the end of our appointment to submit to your insurance for possible reimbursement. Please contact me to schedule your appointment.
