Option 1: Insurance

I am in-network with the following insurance plans: Aetna (PPO, POS, OAP, Select), Blue Cross Blue Shield/Anthem (PPO, EPO), Cigna (PPO, POS, OAP, EPO), Health Net of CA (PPO, Ambetter PPO, Ambetter HMO), Oscar, Prime Healthcare EHP, and United Healthcare (PPO, EPO, HMO). If you have any of these insurances, please text or email me to request an appointment.

Option 2: Self Pay

If you don't have any of the insurances listed above, your appointment will be out-of-pocket. I will provide a Superbill for you at the end of our appointment to submit to your insurance for possible reimbursement. Please text or email me to request an appointment.

Home Visit - Initial: $200

Home Visit - Follow Up: $175

Telehealth - $175

*Payment is required at the time of booking the appointment.
