  • All Tricare West Prime or Tricare Select plans (Includes Reserve, Retiree, Young Adult, etc.)

*Must be at least 27 weeks pregnant; Tricare Overseas and Tricare East NOT covered.

*Home Visit Travel Fee for Tricare: $50

United Healthcare / UMR

  • All commercial plans (including "Oxford, Empire").

Health Net of CA

  • PPO

  • Ambetter PPO

  • Ambetter HMO

  • HMO Fee-for-Service (No Assigned Medical Group)

  • Other HMOs (Pre-Authorization needed)

Anthem Google

  • PPO - Subscriber must be employee of Google

The Lactation Network (TLN)

I am partnered with TLN and they are in-network with most Blue Cross PPO, Blue Shield PPO, Anthem PPO, VA CCN, as well as most plans that utilize Multiplan and PNOA.

Self Pay

If you don't have any of the insurances listed above, your appointment will be out-of-pocket. I will provide a Superbill for you at the end of our appointmemt to submit to your insurance for possible reimbursement,

Home Visit - Initial: $200

Home Visit - Follow Up: $175

Telehealth - $175
