*Must be at least 27 weeks pregnant; Tricare Overseas NOT covered.

*Home Visit Travel Fee for Tricare: $50 up to 15 miles.

United Healthcare / UMR / Empire

The Lactation Network (TLN)

I am partnered with TLN and they are in-network with most Blue Cross PPO, Blue Shield PPO, Anthem PPO, VA CCN, as well as most plans that utilize Multiplan and PNOA.

Self Pay

If you don't have any of the insurances listed above, your appointment will be out-of-pocket. I will provide a Superbill for you at the end of our appointmemt to submit to your insurance for possible reimbursement,

Home Visit - Follow Up: $175

Telehealth - $175
